Paraphilias: definition, diagnosis and treatment

Abstract
"There is a great deal of controversy concerning paraphilia, and defining what is normal versus deviant or disordered, given that this is to some degree dependent on cultural views of acceptability. In this article, we outline these issues and describe recent progress in diagnosing and treating paraphilias."
"Conclusion
"Delineating what is normal versus deviant or disordered sexuality is one of the biggest challenges when using the term paraphilia. The definitions under paraphilia within the DSM have been highly debated and highly controversial [22], and, given the lack of change to definitions and diagnostic criteria in the real sense, this would seem set to continue. The malleability of sexual pleasure across time and cultures creates problems for those defining and diagnosing paraphilia and the efficacy of treatment of paraphilias synonymous with sexual offending is inconclusive. The ability to imprison and/or commit an individual based on their future “dangerousness” using a mental disorder or psychiatric criterion as justification [35,36] may potentially violate due process rights [37]. At the heart of the issues still lies the much debated question: “what justifies the classification of a source of sexual pleasure or a type of sexual activity as a mental disorder” ([22] p.195)?"

American Psychiatric Association holds symposium on removal of paraphilias from DSM
"At its annual convention in May, the American Psychiatric Association (APA) held a symposium to discuss removing gender identity disorder and the paraphilias from the APA's Diagnostic and Statistical Manual of Mental Disorders (DSM). Paraphilias are defined as recurrent and intense sexual feelings or behavior which involve nonhumans, children, or nonconsenting adults, or the suffering or humiliation of oneself or one's partner.1 They include pedophilia, exhibitionism, fetishism, transvestism, voyeurism, and sadomasochism.
The convention was held May 19 in San Francisco. At the symposium, psychiatrist Charles Moser of the Institute for the Advanced Study of Human Sexuality and Peggy Kleinplatz of the University of Ottawa presented a paper entitled "DSM-IV-TR and the Paraphilias: An Argument for Removal.""

The fifth edition of the American Psychiatry Association’s Diagnostic and Statistical Manual of Mental Disorders (DSM) is due out in a few days – the bible of those who would seek to categorise us all under a suitable ‘mental health’ label. This bible was responsible for categorising ‘nicotine deprivation’ as category 292.0 prompting my post warning that nuclear submarines were about to go to sea with a crew comprised of 40% officially suffering from a ‘mental illness’ – a comforting thought!"

British psychologists are to say that current psychiatric diagnoses such as bipolar disorder are useless.

APA rejects "hebephilia," last standing of three novel sexual disorders
"In a stunning blow to psychology's burgeoning sex offender processing industry, the Board of Trustees of the American Psychiatric Association rejected the proposed diagnosis outright, not even relegating it to an appendix as meriting further study, its proponents' fall-back position.
The rejection follows the failure of two other sexual disorders proposed by the DSM-5's paraphilias subworkgroup. These were paraphilic coercive disorder (or a proclivity toward rape) and hypersexuality, an inherently hard-to-define construct that introduced the committee members' value judgments as to how much sex is within acceptable limits.
After abandoning those two disorders, the subworkgroup clung tenaciously to a whittled-down version of its proposed expansion of pedophilia to cover sexual attraction to early pubescent youngsters (generally in the age range of 11-14), ignoring widespread opposition from both within and outside of the APA."

Drugmakers tackle health crisis of child sex abuse
"Leading drugmakers will help set up psychological centers for victims of child sex abuse in an effort to counter the [alleged] long-term health problems suffered by victims.
Roche, Merck and GlaxoSmithKline are to join forces with Harvard Medical School, the Mayo Clinic and others to improve medical education and accelerate the early identification of victims [of what, though?].
Sexual exploitation when a child can [?] saddle a person with life-long illnesses and a range of mental problems [really? where is the causal proof/evidence?]
"For the first time in history, the healthcare industry [sic] is coming together to attack the scourge [sic] of child sexual abuse and exploitation as a public health issue," Franz Humer, chairman of Roche and ICMEC, said on Thursday.
The 25-member committee, which is funded via donations and by governments, will go beyond only providing drugs and plans to develop an "action plan", including undertaking epidemiological research and identifying gaps in the treatment of victims.
At least one in five girls and one in 10 boys are victims [sic] of sexual abuse before they reach the age of 18, but only one in three cases is reported [how do you know, Ernie?], according to the International Centre for Missing and Exploited Children (ICMEC).
Studies have shown that victims of sexual abuse often [rarely, that will be] suffer mental and physical health problems, including depression, anxiety, eating and sleeping disorders, and a higher risk of death from heart disease, diabetes and cancer. [where is the causal proof/evidence?]
While governments have made progress in enacting laws to protect children [sic], the problem is still growing, prompting the need for a coordinated global effort and an industry-wide approach, ICMEC president Ernie Allen said.
"This is a problem of hidden victims," Allen told a news conference. "The key challenge is to awaken the world.""
